You will receive a survey invite link via email from surveys@safeandreliablecare.com starting January 13th.
The survey invite email will contain a personalized link for you to take the survey. Please do not forward your link to others.
Alternatively, if you cannot find your survey invite email, you can go directly to https://surveys.us.safeandreliablecare.com/s/umms and use your employee id to take the survey. For physicians and providers who do not have UMMS employee ids, your passcode is your NPI number.
If you are a new colleague as of the beginning of January or are on LOA, you do not need to take the survey. If you are unsure whether or not you received an invitation and believe you should have, check with your supervisor for assistance.
If you were invited to take the survey, but do not have an email on file (or if you cannot find your survey invite email) you can go directly to https://surveys.us.safeandreliablecare.com/s/umms to access the survey. Use your UMMS employee id or contractor id (e.g., Crothall, Morrison, etc.) to take the survey. For physicians and providers who do not have UMMS employee or contractor ids, your passcode is your NPI number.
No. We recommend using Google Chrome, but all modern browsers are supported. If you believe you are experiencing a browser or network firewall issue accessing the survey, please email surveysupport@safeandreliablecare.com.
On average the full SCORE survey takes about 10-12 minutes to complete. If you are a nurse, it may take a little longer due to the inclusion of Magnet-related items.
You must submit your survey response by February 15th, 2025 at midnight.
Your survey will automatically save in the background every 15 seconds. You can leave the survey page open to return to at a later time or close your browser and later click on your link to continue where you left off. You may return to the survey as many times as you need to until you have submitted your response. Once you have answered all questions, please click on the "Submit" button at the bottom of the survey.
No. You can leave your survey page open for hours or days without worrying about your survey session timing out. Your survey will automatically save in the background every 15 seconds. You can leave the survey page open to return to later or close your browser and later click on your link to continue where you left off.
No. All questions are optional. If you feel that you cannot answer a question, you may leave it blank or, for some questions, mark N/A. Marking N/A is counted the same as if you left the answer blank.
The survey responses are confidential. The survey vendor administering the survey, Vizient Safe and Reliable Healthcare, will not tell UMMS how you answered the survey individually. Your individual response is not anonymous to the survey platform as it is necessary to track which email address needs a reminder link to the survey sent, and your response needs to be tied to a department and team. But the individual response mapping is stored with Vizient Safe and Reliable Healthcare only and not sent to UMMS. Rules are in place to ensure that results are only displayed in the aggregate. No reports are generated for groups of individuals fewer than 5 respondents to protect your individual responses.
The SCORE survey is available in both English and Spanish.
Once you have submitted your survey response you will no longer receive reminder emails to take the survey. If you see a reminder email, click on the link in the email and double-check that it says you have submitted your survey.
If your reminder link loads a blank survey, go back to your original survey invite and check to see if you are getting the survey at two different email address. If so please email surveysupport@safeandreliablecare.com to reach out to Support to let us check if you were invited to take the survey twice using two different emails.
No. Your supervisor will receive daily response rates letting them know how many people on their team have taken the survey but they will not receive a report on who has taken or not taken the survey. Your responses are confidential. Your individual responses will not be shared with your supervisor. Aggregate reports will be generated but only for groups with 5 or more responses to protect your individual response.
Yes. In order to be a valid survey and avoid acquiescence bias, some questions must be negatively worded. It's also easier to answer some questions that are negatively worded. Please read each domain question and indicate how much you agree or disagree with each question.
